Parents have to complete the Request for Baptism Form (both parents have to sign it at the bottom), attach a copy of the child’s birth certificate, and email it to secretaries@sacredheartcalgary.ca
Parents have to give the Godparents Reference Letter to the Potential Godparents. Godparents have to take that Letter/Form to the parish where they belong, meet with the Priest or Deacon, and have them complete the form. Once the letter/form is completed, parents can scan and email it to secretaries@sacredheartcalgary.ca Once Sacred Heart Parish receives all these documents, we can book the date for the baptisms.

Who can serve as a Godparent:
- Practicing Catholic (explanation):
- A person who goes to Church on Sundays
- Receives Communion
- Goes regularly to confession
- Has the Sacrament of Confirmation
- If the potential Godparent is married, he/she has to be married in the Catholic Church (if he/ she is married civilly or has a common-law spouse, this person can not serve as a Godparent)
- You do not have to have two Godparents, you can have only one, but if you have two, they have to be a male and a female

Please note that:
- Baptisms are discouraged during the Lenten season.
- Families from other parishes are required to obtain written permission from their respective pastors in order to baptize their child at Sacred Heart Church
