Sacraments Record Request

A sacramental record indicates an individual’s canonical status – their status in the Catholic Church as it relates to the sacraments they have received. This includes Baptism, Confirmation, Marriage, and Decree of Nullity. Please note that we only have records for sacraments that were received in a
Roman Catholic parish within the boundaries of the Diocese of Calgary.

We are pleased to introduce a new, streamlined process for requesting sacramental records. All requests are to be submitted through the Diocese’s online application form. This ensures that requests are processed efficiently, communication with parishes is clear, and families receive support in a timely manner.

Please note that while the RCDCA will make every effort to locate requested records, we cannot guarantee the availability of a record. Please allow a minimum of two weeks for requests to be processed.

Sacraments Received in the Diocese of Calgary

Step 1: Prepare Information
Applicants should have valid documentation to verify sacramental record information. The online application will specify what is required.

Step 2: Complete Online Application
Sacramental record requests are to be submitted through the online application form. No information from sacramental records is to be provided over the phone or by other electronic means. No fee is to be charged by the parish.

Step 3: Diocese Reviews Application
The Diocese will review the application request, and, if applicable, will forward to the relevant parish for processing and issuing a copy of the record.

Sacraments Not Received in the Diocese of Calgary or in another Catholic diocese or Eastern Catholic Church

  • Step 1: Contact the Parish
Applicants must first contact the parish where the sacrament took place. If the parish cannot be contacted, it is necessary to contact the respective Diocese, Eparchy or Ordinariate for the region.

  • Step 2: Unable to Find Records
If the parish or respective Diocese cannot assist, applicants are to complete the online application.

  • Step 3: Submit Affidavit
The Diocese will review the record request, and will process accordingly, if applicable, to an affidavit. Only the Office of the Chancellor is able to provide and complete affidavits. The local parish will be involved as needed.

Please note that Alberta Vital Statistics’ birth, marriage, and death records eventually become historical records and are only available at the Provincial Archives of Alberta.

The Office of the Chancellor and the Archives are available to assist throughout this process by contacting sacramentalrecords@calgarydiocese.ca
